Why Hydration and Tissue Integrity Are Closely Linked in Pressure Ulcer Prevention

The upcoming Nutrition & Hydration Week (16–22 March 2026) serves to highlight the importance of promoting better nutritional and hydration care across health and social care settings, both in the UK and globally. 

And, whilst hydration is often associated with general wellbeing, its role in maintaining tissue integrity and reducing pressure ulcer risk certainly deserves equal attention because, for individuals vulnerable to pressure damage, hydration is not a secondary consideration – t is a fundamental component of prevention.

Hydration and the Structure of Healthy Skin

Tissue integrity depends on balanced fluid levels within the body, where adequate hydration supports skin elasticity, cellular function, and circulation; all essential factors in helping tissue tolerate sustained pressure.

Well-hydrated skin is more supple and resilient, and therefore is better able to withstand friction and shear, as well as being more capable of recovering from minor stress. 

When hydration levels fall, however, the skin can become dry, fragile, and less tolerant of compression and in individuals with reduced mobility, this decreased tolerance can significantly increase the risk of pressure-related tissue damage.

When Dehydration Compromises Tissue Integrity

Dehydration in care settings is often gradual and may not be immediately obvious, and individuals who rely on assistance for fluid intake, who have swallowing difficulties, or who experience cognitive impairment may be particularly vulnerable.

Even mild dehydration can affect circulation and oxygen delivery to tissues where, over time, this can impair the skin’s natural defence mechanisms and reduce its ability to cope with sustained pressure. Areas such as the heels, sacrum, and hips – already exposed to loading – may become increasingly susceptible to breakdown.

Where pressure ulcers are already present, inadequate hydration may also delay healing by limiting cellular repair processes.

Hydration Within a Holistic Prevention Strategy

Pressure redistribution remains essential in protecting vulnerable individuals. Specialist cushions, mattresses, and positioning systems reduce sustained loading and minimise mechanical stress on high-risk areas.

However, pressure care equipment works most effectively when tissue integrity is supported from within. Hydration strengthens the skin’s resilience, improves elasticity, and enhances its ability to recover from unavoidable pressure exposure.

By integrating hydration monitoring into routine pressure ulcer risk assessments, care teams can adopt a more comprehensive approach, addressing both external and internal contributors to tissue damage.

Strengthening Tissue Integrity Through Everyday Practice

Encouraging regular fluid intake, supporting individuals who require assistance, and monitoring for early signs of dehydration are practical measures that can significantly influence outcomes.

Hydration should be viewed as a core element of tissue viability planning, rather than an isolated nutritional concern. When fluid balance is prioritised alongside repositioning and appropriate support surfaces, the result is a stronger, more resilient defence against pressure ulcers.
