Hospital readmissions within 30 days of being discharged stood at approximately 14% here in the UK in 2022/23 and, for the vast majority of patients, staying out of hospital once successfully discharged is imperative to their ongoing physical and mental wellbeing; able to see through the rest of their recovery in the comfort of their own homes and/or surrounded by their loved ones.
